Separate documentation gaps from amount breaches. A missing boarding pass is usually fixed with a process reminder. A repeated entertainment overage needs a policy rewrite or a harder approval gate.
Ask for severity labels that match your risk appetite: informational, requires recovery, or requires policy change. Without those labels, leadership treats every yellow highlight as equal and the remediation meeting stalls.
Close the loop by assigning each pattern an owner and a date. Reviews that end as PDFs in a shared folder tend to reappear, nearly unchanged, in the next sampling cycle.
